- Visit the EssentiaHealth.org website and use the e-visit functionality. This will connect you to a medical provider who can discuss your symptoms and determine the best course of action that you should pursue to address your illness.
- Call the toll free hotline that Essentia has established. The number is (833) 494-0836.
- Call the Minnesota Department of Health’s public COVID-19 number which is (651) 201-3920.
- You can use the CDC’s Coronavirus Self-Checker on-line at https://www.cdc.gov/coronavirus/2019-ncov/symptoms-testing/symptoms.html.
- Completing the Telehealth Appointment request using the web based form at https://bit.ly/ECHCTelehealthAppointment, or
- Calling our voicemail at (218) 365-5678 and leaving your name, the date that you would like to have a Telehealth appointment for and phone number to reach you.

Our Services
The Ely Community Health Center is proud to offer free, non-emergency healthcare to our Ely area neighbors. Our clinic is open every Thursday evening from 5:30-7pm. We are located 33 E Chapman Street, Ely MN 55731.
Some of the services we offer include:
We now provide basic oral healthcare services including regular cleanings by appointment. We also provide fluoride varnish treatments on Thursdays during our regular clinic hours. Having these fluoride varnish treatments extends the time period between normal oral health care for families and individuals who do not have access to normal oral health care.

- Basic lab and X-ray services through an agreement with Ely-Bloomenson Community hospital
- Medication assistance
- Help with applications to public insurance options such as Medicaid and MNsure
- Community referrals and connections
- MNSure navigation services and help applying for health insurance including public programs

Our History |
A free clinic operated in Ely from March 2011 until June 2015 as one of four free clinics operated by the non-profit, Project Care. During this time we served primarily uninsured individuals in our community two evenings per month. Because of a budget shortfall, the Project Care board voted to close the Ely free clinic in May of 2015.
A group of Ely free clinic volunteers felt that the need for a free clinic in Ely continues, especially if we were to expand our services to both those who are underinsured as well as those who are uninsured. In addition, we hope to form close alliances with existing service organizations and individuals to enhance access to a variety of health care services for our clients. |
